Creating Item from within Initiative

Hi,

I am trying to create a item from option "Create Item" from Initiative,

When I try saving the item, I am getting the below error,

"Internal error. Diagnosis The call of method GET_CHILDREN failed. System Response Procedure Contact your system administrator. Procedure for System Administration"

Can anyone let me know why is this error shown.
